LS Consulting Solutions LLC is a limited liability company organized under the laws of the State of California providing specialized documentation support services to licensed home health agencies and residential care facilities. We provide pre-submission quality assurance reviews for home health agencies nationwide and LIC form preparation and binder organization services for residential care facilities in California.

Scope of this policy

This Privacy Policy applies to personal information collected through the LS Consulting Solutions LLC website located at lsconsultingsolutions.com, including any subdomains, contact forms, inquiry forms, and onboarding forms hosted on or linked from the site.

Critical distinction

This Privacy Policy does not govern Protected Health Information (PHI) received in connection with home health QA services, which is governed by the applicable Business Associate Agreement and HIPAA. It also does not govern resident information received in connection with RCFE documentation services, which is governed by the applicable Confidentiality Agreement. 3.1 — Information you provide directly

When you complete a contact, inquiry, or onboarding form on this site, we may collect:

• Full name and professional title

• Name and license number of the agency or facility you represent

• Business email address and telephone number

• State and county of operation

• Service type of interest

• Any additional information voluntarily provided in message fields

3.2 — Information collected automatically

When you visit the site, certain technical information is collected automatically, including IP address, browser type, operating system, pages visited, and date and time of visit.

3.3 — Information we do not collect through this website

Do not submit PHI or resident information through this website

Patient Protected Health Information and RCFE resident information must never be submitted through any form or channel on this site. PHI is exchanged only through HIPAA-compliant channels established after BAA execution. Resident information is exchanged only through secure channels established after Confidentiality Agreement execution.

How we use your information

Personal information collected through this site is used solely for legitimate business purposes including:

• Responding to inquiries and service onboarding submissions

• Evaluating whether an agency or facility qualifies for our services

• Communicating with prospective and current clients regarding the service

• Preparing, negotiating, and administering service agreements and information protection agreements

• Delivering our documentation support services

• Maintaining internal business records

• Complying with applicable federal and California state law

• Detecting, preventing, or investigating fraud or security incidents

LS Consulting Solutions LLC does not use personal information for third-party advertising, behavioral marketing, or any purpose unrelated to the provision of its services.

How we share your information

LS Consulting Solutions LLC does not sell, rent, lease, or transfer personal information to any third party for commercial purposes under any circumstances.

Non-client inquiry submissions are retained for no more than twenty-four (24) months following the date of last contact, absent an ongoing business relationship. This site and our services are directed exclusively to licensed healthcare professionals, home health agency owners, administrators, and residential care facility operators — all of whom must be adults.
